Overview

Fingolimod is an oral small molecule immunomodulator used to treat relapsing forms of multiple sclerosis (MS), including clinically isolated syndrome, relapsing-remitting disease, and active secondary progressive disease. It acts as a sphingosine 1-phosphate receptor modulator by binding to sphingosine 1-phosphate receptors (S1PRs), particularly subtypes 1, 3, 4, and 5. This mechanism prevents lymphocytes from exiting lymph nodes into the peripheral circulation, thereby reducing immune-mediated inflammation in the central nervous system associated with MS. Fingolimod was developed by Novartis and was first approved for use in MS in 2010[5][6][1].
