Overview

FIRINOX is a modified chemotherapy regimen developed by Wakayama Medical University for the neoadjuvant treatment of borderline resectable pancreatic ductal carcinoma. It is a simplified version of the standard FOLFIRINOX protocol, consisting of oxaliplatin, irinotecan, and continuous infusion fluorouracil (5-FU). The modification involves the removal of leucovorin (folinic acid) and the 5-FU bolus, alongside a dose reduction of irinotecan to 150 mg/m2. These changes are intended to mitigate the high rates of grade 3/4 neutropenia and gastrointestinal toxicities associated with standard FOLFIRINOX while maintaining therapeutic efficacy in the preoperative setting.
