First-line TB treatment refers to the standard combination drug regimen used for the initial management of active, drug-susceptible tuberculosis. The canonical regimen consists of four drugs—isoniazid, rifampicin (also known as rifampin), pyrazinamide, and ethambutol—administered together during an intensive phase (typically two months), followed by a continuation phase with isoniazid and rifampicin for an additional four months. These drugs act synergistically to kill actively replicating Mycobacterium tuberculosis and prevent resistance development. Isoniazid inhibits mycolic acid synthesis in the bacterial cell wall; rifampicin inhibits DNA-dependent RNA polymerase; pyrazinamide disrupts membrane transport and energy production in acidic environments; ethambutol impairs cell wall arabinogalactan synthesis. This combination is highly effective against most strains of M. tuberculosis when taken as prescribed[1][2][5][6].
