Overview

Fish oil + metformin is a non-branded combination of two agents—fish oil (omega-3 polyunsaturated fatty acids) and metformin—used together primarily in research and clinical practice to address metabolic disorders. Fish oil contains eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA), which are known for their triglyceride-lowering effects, anti-inflammatory properties, and cardiovascular benefits. Metformin is a biguanide oral antihyperglycemic agent that improves insulin sensitivity, decreases hepatic glucose production, and enhances peripheral glucose uptake. The combination has been studied in populations with metabolic syndrome, type 2 diabetes mellitus, or polycystic ovary syndrome (PCOS). Clinical trials suggest that the addition of high-dose fish oil to metformin therapy can further reduce fasting and postprandial plasma triglycerides as well as ApoB-lipoprotein remnants compared to either agent alone[3][4][5]. This may be particularly beneficial for patients with dyslipidemia associated with PCOS or diabetes. Mechanistically: - Fish oil acts by reducing hepatic very-low-density lipoprotein (VLDL) synthesis/secretion and increasing clearance of triglyceride-rich particles. - Metformin activates AMP-activated protein kinase (AMPK), reduces hepatic gluconeogenesis, increases insulin sensitivity in muscle/adipose tissue, modulates gut microbiota/metabolites such as 3-hydroxy medium chain fatty acids[6], and exerts mild antilipidemic effects. No significant drug-drug interactions have been reported between these agents[7][9]. The combination is not available as a fixed-dose pharmaceutical product but may be used off-label or in clinical studies.
