Fish oil triglyceride is a formulation of fish oil in which the omega-3 fatty acids—primarily eicosapentaenoic acid (EPA) and docosahexaenoic acid (DHA)—are present as natural triglyceride esters. It is used as a dietary supplement and as a component in parenteral nutrition, and certain prescription formulations are approved for treatment of severe hypertriglyceridemia (≥500 mg/dL). The mechanism of action involves reduction of serum triglyceride levels through several proposed routes: omega-3 fatty acids may influence lipid metabolism by shifting cellular metabolism toward increased fatty acid oxidation and reduced triglyceride synthesis, promoting apolipoprotein B degradation, accelerating clearance of very-low-density lipoprotein (VLDL) particles, and modulating inflammatory processes through the production of resolvins and protectins. Fish oil triglyceride is most often administered orally, though parenteral preparations exist for use when oral or enteral nutrition is not possible[1][7].
