Overview

FLAG+IDA is a multi-agent chemotherapy regimen used primarily for the treatment of acute myeloid leukemia (AML) and high-risk myelodysplastic syndromes (MDS), especially in relapsed or refractory cases. The regimen consists of four drugs:\n- Fludarabine (a purine analog antimetabolite)\n- Cytarabine (also known as Ara-C; a pyrimidine analog antimetabolite)\n- Idarubicin (an anthracycline antibiotic that intercalates DNA and inhibits topoisomerase II)\n- Granulocyte colony-stimulating factor (G-CSF; typically filgrastim), which stimulates bone marrow to produce neutrophils.\n\nFludarabine enhances the accumulation of the active metabolite of cytarabine in leukemic cells. G-CSF is administered to synchronize leukemic cells into a more vulnerable phase of the cell cycle for chemotherapy. Idarubicin adds further anti-leukemic activity by inducing DNA damage in rapidly dividing cells[1][2][6]. This combination achieves high rates of complete remission in AML and MDS patients but is associated with significant myelosuppression and risk of infection[2][4].
