Overview

Florensocatib (CHF10196) is an oral small molecule inhibitor of dipeptidyl peptidase 1 (DPP1), also known as cathepsin C, currently in Phase III clinical development by Chiesi Farmaceutici. DPP1 is a lysosomal cysteine protease responsible for the activation of neutrophilic serine proteases (NSPs), including neutrophil elastase, proteinase 3, and cathepsin G, during neutrophil maturation in the bone marrow. In patients with non-cystic fibrosis bronchiectasis (NCFB), excessive NSP activity in the airways drives chronic inflammation, mucus hypersecretion, and progressive lung damage. By inhibiting DPP1, florensocatib prevents the activation of these pro-inflammatory proteases, thereby reducing airway inflammation and the frequency of pulmonary exacerbations. The drug is being evaluated in large-scale clinical trials, such as the FABLE study, to assess its efficacy and safety in both adult and adolescent populations.
