FLOT or FOLFOX-6

FLOT and FOLFOX-6 are established multi-drug cytotoxic chemotherapy regimens used primarily in the treatment of gastrointestinal malignancies, including esophageal and gastroesophageal junction (GEJ) cancers. FLOT is a four-drug perioperative regimen consisting of fluorouracil (5-FU), leucovorin (folinic acid), oxaliplatin, and docetaxel. FOLFOX-6 (often administered as modified mFOLFOX-6) is a three-drug regimen consisting of fluorouracil, leucovorin, and oxaliplatin. These regimens target tumor cells through multiple mechanisms: oxaliplatin induces DNA damage via platinum-based crosslinking; fluorouracil, potentiated by leucovorin, acts as an antimetabolite inhibiting thymidylate synthase; and docetaxel (in the FLOT regimen) disrupts microtubule dynamics. In the context of the PULSO Trial (NCT06043570) at the Medical College of Wisconsin, these regimens serve as the chemotherapy backbone for patients undergoing chemoradiation for esophageal cancer to evaluate the impact of Pulsed Low-Dose-Rate (PLDR) radiation on esophagitis.
