This is a combination of five antifungal agents—fluconazole, itraconazole, posaconazole, terbinafine, and voriconazole. All except terbinafine are triazole antifungals; terbinafine is an allylamine. These drugs act by inhibiting key enzymes in the fungal cell membrane synthesis pathway (primarily ergosterol biosynthesis), leading to disruption of cell membrane integrity and ultimately fungal cell death or growth inhibition. Combination antifungal therapy (CAF) has been explored to broaden the spectrum of activity, enhance efficacy through potential synergy, delay resistance development, and reduce toxicity by allowing lower doses of individual drugs. However, such combinations are not standard clinical practice due to concerns about antagonism between agents (especially among azoles), increased risk for drug-drug interactions and toxicity, and lack of robust evidence from randomized trials supporting their routine use[1][2][3]. This specific five-drug combination is not a recognized or approved regimen but represents an extreme example of CAF.
