Overview

This is a combination regimen consisting of four drugs: fludarabine, cyclophosphamide, rituximab, and lenalidomide. Fludarabine is a purine analog that inhibits DNA synthesis by interfering with DNA polymerase and ribonucleotide reductase. Cyclophosphamide is an alkylating agent that crosslinks DNA strands, leading to cell death. Rituximab is a monoclonal antibody targeting CD20 on B lymphocytes, resulting in immune-mediated cell lysis. Lenalidomide is an immunomodulatory agent that alters cytokine production, enhances T-cell and NK-cell activity, and induces degradation of specific transcription factors via the cereblon E3 ubiquitin ligase complex[1][4][6][8]. This combination has been investigated primarily for the treatment of chronic lymphocytic leukemia (CLL), aiming to improve response rates by combining cytotoxic chemotherapy with targeted immunotherapy and immune modulation[5][8].
