Fludarabine + busulfan is a combination chemotherapy regimen primarily used as a conditioning treatment before allogeneic hematopoietic stem cell transplantation, especially in patients with acute myeloid leukemia (AML) or myelodysplastic syndromes (MDS)[1][4]. Fludarabine is a purine analog antimetabolite that inhibits DNA synthesis by interfering with DNA polymerase alpha, ribonucleotide reductase, and DNA primase[3][5]. Busulfan is a bifunctional alkylating agent that cross-links DNA strands, leading to inhibition of DNA replication and transcription and ultimately causing cell death[6][8]. The combination provides effective immunosuppression and cytoreduction with reduced toxicity compared to traditional regimens. Both drugs are small molecules administered intravenously in this setting.
