Overview

The FCL regimen is a combination therapy consisting of the purine analog fludarabine, the alkylating agent cyclophosphamide, and the immunomodulatory drug (IMiD) lenalidomide. This specific combination was developed and evaluated in Phase I/II clinical trials (such as the LLC0606 study) by the Gruppo Italiano Malattie EMatologiche dell'Adulto (GIMEMA) for the treatment of patients with relapsed or refractory chronic lymphocytic leukemia (CLL). The regimen builds upon the standard fludarabine and cyclophosphamide (FC) backbone by adding lenalidomide, which acts as a molecular glue to enhance immune-mediated anti-tumor activity and exert direct anti-proliferative effects on malignant B-cells. Lenalidomide's primary mechanism involves binding to the Cereblon E3 ubiquitin ligase complex, leading to the degradation of transcription factors Ikaros and Aiolos.
