This is a combination chemotherapy regimen consisting of fludarabine, cytarabine, and idarubicin. It is primarily used in the treatment of acute myeloid leukemia (AML), particularly for relapsed/refractory cases or as induction therapy in newly diagnosed patients with high-risk features. Fludarabine and cytarabine are antimetabolite chemotherapeutic agents that inhibit DNA synthesis by interfering with DNA replication and repair, leading to cell death in rapidly dividing cells. Idarubicin is an anthracycline antibiotic that intercalates into DNA and inhibits topoisomerase II, resulting in inhibition of DNA replication and repair as well as generation of free radicals that cause further cellular damage. The combination exploits synergistic effects to maximize anti-leukemic activity[1][3][5]. This regimen may be referred to as FLA-IDA or m-FLAI (modified dosing) depending on the protocol.
