This is a combination therapy consisting of fludarabine, a purine analog antimetabolite chemotherapeutic agent, and darbepoetin alfa, an erythropoiesis-stimulating agent. Fludarabine inhibits DNA synthesis by interfering with DNA polymerase alpha, ribonucleotide reductase, and DNA primase after intracellular conversion to its active triphosphate form. It is primarily used in the treatment of B-cell chronic lymphocytic leukemia (CLL) and other hematologic malignancies[6][8]. Darbepoetin alfa is a recombinant erythropoietin analog that stimulates red blood cell production by binding to the erythropoietin receptor on erythroid progenitor cells in the bone marrow; it is used to treat anemia associated with chemotherapy or chronic kidney disease[9][5]. The combination has been studied in older patients with CLL and comorbidities to address both cancer control (via fludarabine) and chemotherapy-induced anemia (via darbepoetin)[1][4].
