Overview

FAD is a combination chemotherapy regimen consisting of fludarabine, doxorubicin (Adriamycin), and dexamethasone. It is primarily investigated as a first-line therapy for patients with newly diagnosed peripheral T-cell lymphoma (PTCL). Fludarabine is a purine nucleoside analog that interferes with DNA synthesis by inhibiting DNA polymerase and ribonucleotide reductase. Doxorubicin is an anthracycline antibiotic that works by intercalating into DNA and inhibiting topoisomerase II, leading to DNA damage and cell death. Dexamethasone is a synthetic glucocorticoid that provides anti-inflammatory and immunosuppressive effects while also inducing apoptosis in lymphoid cells. This specific protocol, often utilizing an oral formulation of fludarabine, was studied by Fudan University to evaluate its efficacy and safety in aggressive T-cell lymphomas.
