Fludarabine + melphalan is a combination chemotherapy regimen used primarily as a reduced-intensity conditioning (RIC) protocol before allogeneic hematopoietic stem cell transplantation (HSCT), especially in patients with acute myeloid leukemia (AML), myelodysplastic syndromes (MDS), and other hematologic malignancies. Fludarabine is a purine analog that inhibits DNA synthesis by interfering with DNA polymerase, ribonucleotide reductase, and DNA primase, leading to inhibition of cell proliferation. Melphalan is an alkylating agent that crosslinks DNA strands, resulting in cytotoxicity and apoptosis of rapidly dividing cells. The combination leverages the immunosuppressive effects of both agents to facilitate engraftment while providing antitumor activity[1][2]. This regimen has become widely used due to its efficacy in disease control with reduced toxicity compared to traditional high-dose regimens.
