This is a reduced-intensity conditioning (RIC) regimen developed by the University Hospital Tuebingen for patients undergoing haploidentical hematopoietic stem cell transplantation (HCT). The protocol is specifically designed for high-risk patients with acute myeloid leukemia (AML) or acute lymphoblastic leukemia (ALL) in complete remission who lack a matched donor. It combines three chemotherapy agents—fludarabine (a purine analog), thiotepa, and melphalan (both alkylating agents)—with Thymoglobuline (anti-thymocyte globulin), a polyclonal antibody used for T-cell depletion. This combination aims to provide sufficient immunoablation and myeloablation to allow for the engraftment of CD3/CD19-depleted stem cell grafts while minimizing regimen-related toxicity compared to standard myeloablative conditioning.
