Fluoroethoxybenzovesamicol (FEOBV), specifically the [18F]-labeled enantiomer, is a radiopharmaceutical small molecule that acts as a highly selective ligand for the vesicular acetylcholine transporter (VAChT). It is used as a positron emission tomography (PET) imaging tracer to provide a direct in vivo measurement of presynaptic cholinergic terminal density and integrity in the central nervous system. Unlike previous cholinergic biomarkers that measure both pre- and post-synaptic expressions, FEOBV specifically targets VAChT, making it a valuable tool for studying cholinergic denervation in neurodegenerative conditions such as Parkinson's disease, Alzheimer's disease, Down syndrome, and progressive supranuclear palsy. It is also utilized in research to evaluate the effects of cognitive training on cholinergic network health in aging populations.
