Fluoromisonidazole F-18 is a radiopharmaceutical used for Positron Emission Tomography (PET) imaging to detect and quantify hypoxia, particularly in tumors. It is a 2-nitroimidazole molecule labeled with the positron-emitting isotope fluorine-18. The drug selectively binds to macromolecules within hypoxic cells. In low-oxygen environments, its nitro group is reduced and then irreversibly binds to intracellular macromolecules, leading to its accumulation. In contrast, in normoxic (non-hypoxic) cells, the reduced form can be re-oxidized and diffuses out of the cell. This differential accumulation allows for the visualization of hypoxic regions using PET imaging. It is utilized in various cancers, including lung, brain, head-and-neck, breast, soft tissue sarcoma, liver, gastrointestinal, and uterine cancers, as well as in myocardial ischemia, to assess tumor hypoxia and guide treatment strategies.
