fluorouracil + capecitabine is a combination chemotherapy regimen consisting of two fluoropyrimidine antimetabolites. Fluorouracil (5-FU) is a well-established antineoplastic agent that inhibits DNA synthesis by interfering with thymidylate synthase (TS), while capecitabine is an orally administered prodrug that is enzymatically converted to 5-FU within tumor tissue. This combination is frequently utilized as a backbone chemotherapy in clinical trials, such as the EORTC INNOVATION trial, for the treatment of HER2-positive resectable gastric and gastroesophageal junction (GEJ) adenocarcinomas. By targeting thymidylate synthase and incorporating into RNA and DNA, these agents disrupt tumor cell growth and replication in rapidly dividing cancer cells.
