This is a combination regimen consisting of four drugs: - **Fluorouracil (5-FU):** An antimetabolite that inhibits thymidylate synthase, disrupting DNA synthesis and leading to cell death. - **Leucovorin (folinic acid):** Enhances the binding of 5-FU to thymidylate synthase, increasing its cytotoxicity. - **Oxaliplatin:** A platinum-based chemotherapeutic agent that forms DNA crosslinks and inhibits DNA replication and transcription. - **Sorafenib:** A multikinase inhibitor targeting RAF kinases as well as vascular endothelial growth factor receptors (VEGFR) and platelet-derived growth factor receptor (PDGFR), inhibiting tumor cell proliferation and angiogenesis. This combination has been studied primarily in advanced hepatocellular carcinoma (HCC) using hepatic arterial infusion chemotherapy (HAIC), as well as in metastatic colorectal cancer. The rationale is to combine the cytotoxic effects of FOLFOX with the antiangiogenic/targeted action of sorafenib. Clinical trials have shown mixed results regarding synergism; some studies suggest improved survival compared to monotherapy or standard regimens[1][2][3].
