Overview

Fluoxetine combined with repetitive transcranial magnetic stimulation (rTMS) is an experimental therapeutic approach being evaluated for motor recovery following ischemic stroke. Fluoxetine is a small molecule selective serotonin reuptake inhibitor (SSRI) that inhibits the serotonin transporter (SERT), potentially enhancing neuroplasticity and neurogenesis. In this combination therapy, 1-Hz low-frequency rTMS is applied to the contralesional (unaffected) primary motor cortex to downregulate its excitability, aiming to balance interhemispheric inhibition and facilitate motor function in the paretic limb. This specific protocol was investigated in a Phase 2 clinical trial (NCT02208466) led by Spaulding Rehabilitation Hospital, which demonstrated that the combination of fluoxetine and rTMS led to significantly better upper limb motor outcomes than fluoxetine monotherapy or placebo in patients with hemiparesis following an ischemic stroke.
