Overview

Flurpiridaz F 18 is a cyclotron-produced radioactive diagnostic agent used for positron emission tomography (PET) myocardial perfusion imaging (MPI) in adults with known or suspected coronary artery disease to evaluate for myocardial ischemia and infarction. It is administered intravenously under rest or stress conditions (pharmacologic or exercise). Flurpiridaz F 18 is an analog of the mitochondrial complex I inhibitor pyridaben; it binds to heart tissue with biologically active mitochondria, resulting in higher radioactivity in viable myocardium compared to infarcted tissue. The drug exhibits high myocardial extraction and retention, providing high-resolution images for the assessment of coronary artery disease. The primary mechanism of action involves its uptake by myocardium proportional to blood flow and binding to mitochondrial complex I within heart cells[1][2][3][7].
