Overview

Fluticasone + azithromycin is a non-standard, compounded combination therapy consisting of fluticasone (a synthetic glucocorticoid corticosteroid) and azithromycin (a macrolide antibiotic). This combination has been studied primarily in the context of chronic respiratory diseases, such as chronic cough due to Mycoplasma pneumoniae infection in children and bronchiolitis obliterans syndrome after allogeneic hematopoietic cell transplantation. Fluticasone acts by activating glucocorticoid receptors to reduce inflammation, while azithromycin inhibits bacterial protein synthesis by binding to the 23S rRNA of the bacterial 50S ribosomal subunit. Azithromycin also exhibits immunomodulatory effects that may benefit patients with chronic inflammatory airway diseases. The combination aims to reduce airway inflammation, improve immune function, and enhance treatment efficacy while potentially reducing reliance on systemic corticosteroids[1][3][8].
