Overview

FMT + atezolizumab + bevacizumab is an experimental combination therapy being investigated for the treatment of unresectable hepatocellular carcinoma (HCC). This regimen is specifically designed for patients who have experienced disease progression after receiving first-line therapy with the immune checkpoint inhibitor atezolizumab and the anti-angiogenic agent bevacizumab. The therapy involves the administration of fecal microbiota transplantation (FMT) to modulate the patient's gut microbiome, which is hypothesized to play a crucial role in determining the efficacy of and resistance to immunotherapy. By altering the microbial composition, the treatment aims to 'rescue' the therapeutic response to the resumed atezolizumab and bevacizumab regimen. This approach is currently being evaluated in a Phase 2 study led by the IRCCS Azienda Ospedaliero-Universitaria di Bologna.
