FOLFIRI + bevacizumab is a combination chemotherapy and targeted therapy regimen used primarily for the treatment of metastatic colorectal cancer. The regimen consists of four drugs: - Bevacizumab, a monoclonal antibody that inhibits vascular endothelial growth factor (VEGF), thereby blocking angiogenesis and reducing tumor blood supply. - Fluorouracil (5-FU), an antimetabolite that incorporates into DNA and RNA to inhibit cell division. - Irinotecan, a topoisomerase I inhibitor that prevents DNA from uncoiling and duplicating. - Leucovorin (folinic acid), which enhances the effectiveness of fluorouracil. This combination works synergistically by attacking cancer cells through different mechanisms—chemotherapy agents disrupt DNA synthesis and cell division, while bevacizumab targets tumor vasculature. It is administered intravenously in cycles every two weeks. The primary indication is metastatic colorectal cancer; it may also be used for other gastrointestinal cancers such as appendiceal adenocarcinoma and small bowel adenocarcinoma[1][2][4][5][8].
