Overview

FOLFIRI + FOLFOX + bevacizumab is a sequential chemotherapy treatment strategy evaluated in the ITACa (Italian Trial in Advanced Colorectal Cancer) Phase III trial for the treatment of metastatic colorectal cancer. This regimen combines two standard cytotoxic chemotherapy protocols—FOLFIRI (irinotecan, leucovorin, and 5-fluorouracil) and FOLFOX (oxaliplatin, leucovorin, and 5-fluorouracil)—with bevacizumab, a recombinant humanized monoclonal antibody. Bevacizumab acts as an angiogenesis inhibitor by specifically binding to the vascular endothelial growth factor (VEGF) protein, thereby preventing its interaction with receptors on the surface of endothelial cells. The cytotoxic components work by inhibiting DNA topoisomerase I (irinotecan), inhibiting thymidylate synthase (5-fluorouracil), and forming DNA cross-links (oxaliplatin). The ITACa trial, sponsored by the Istituto Romagnolo per lo Studio dei Tumori (IRST), investigated the optimal sequencing of these agents to improve progression-free and overall survival in patients with advanced colorectal disease.
