Overview

This regimen is a combination of multiple oncology therapies, used investigationally for the treatment of advanced or refractory cancers such as metastatic colorectal cancer. It combines two intensive chemotherapy regimens (FOLFIRI and modified FOLFOX6) with three monoclonal antibodies (panitumumab, ipilimumab, nivolumab) that provide targeted and immunotherapy effects. - **FOLFIRI** comprises folinic acid (leucovorin), fluorouracil (5-FU), and irinotecan. Folinic acid enhances the action of 5-FU, while 5-FU inhibits thymidylate synthase, interfering with DNA synthesis. Irinotecan is a topoisomerase I inhibitor causing DNA breakage during replication[1][3][5][11][13]. - **mFOLFOX6** consists of oxaliplatin, leucovorin (folinic acid), and fluorouracil (5-FU), given in a modified dosing schedule. Oxaliplatin is a platinum-based agent that causes DNA crosslinking and apoptosis. Leucovorin and 5-FU act as above[4][6][8][10][12][14][15]. - **Panitumumab** is a fully human monoclonal antibody that inhibits the epidermal growth factor receptor (EGFR), resulting in inhibition of cell growth and induction of apoptosis in cancer cells. - **Ipilimumab** is a monoclonal antibody that blocks cytotoxic T-lymphocyte-associated protein 4 (CTLA-4), a negative regulator of T-cell activation, thereby enhancing immune-mediated tumor cell killing. - **Nivolumab** is a monoclonal antibody targeting the programmed death-1 (PD-1) receptor, blocking its inhibitory signaling and restoring T-cell activity against tumor cells. This multi-drug combination leverages chemotherapy-induced cytotoxicity, EGFR-targeted inhibition, and dual immune checkpoint blockade for synergistic effects in the management of resistant or advanced cancers.
