FOLFIRINOX is a combination chemotherapy regimen used primarily for the treatment of advanced pancreatic cancer, including metastatic and locally advanced (inoperable) cases. It consists of four drugs: - Folinic acid (leucovorin): a vitamin B derivative that enhances the effects of fluorouracil. - Fluorouracil (5-FU): an antimetabolite that disrupts DNA synthesis in rapidly dividing cells. - Irinotecan: a topoisomerase I inhibitor that prevents DNA from uncoiling and duplicating. - Oxaliplatin: a platinum-based agent that causes DNA crosslinking and damage. The regimen works by targeting cancer cells through multiple mechanisms to inhibit their growth and division. While it has demonstrated improved survival rates compared to other regimens like gemcitabine, it is associated with significant toxicity and is generally reserved for patients with good performance status. It may also be used as neoadjuvant therapy or in certain cases of bowel cancer[1][2][3][4][5].
