Overview

This neoadjuvant treatment regimen, evaluated in the PIOPPO Phase II clinical trial, combines the multi-agent chemotherapy protocol FOLFIRINOX with carbon ion radiation therapy (hadrontherapy) for patients with resectable or borderline resectable pancreatic adenocarcinoma. FOLFIRINOX consists of oxaliplatin (a DNA cross-linker), irinotecan (a topoisomerase I inhibitor), leucovorin (a folinic acid derivative that potentiates fluorouracil), and fluorouracil (a thymidylate synthase inhibitor). Carbon ion therapy utilizes high-LET (linear energy transfer) radiation to deliver a precise dose to the tumor (Bragg peak), offering superior relative biological effectiveness compared to conventional photon-based radiotherapy. The goal of this multimodal approach is to increase the rate of R0 surgical resections and improve relapse-free survival in patients with localized pancreatic cancer.
