FOLFOX4 + bevacizumab is a combination chemotherapy regimen used primarily for the treatment of metastatic colorectal cancer. The regimen consists of FOLFOX4 (a combination of oxaliplatin, fluorouracil [5-FU], and folinic acid [leucovorin]) together with the monoclonal antibody bevacizumab. - **Mechanism of Action:** - Oxaliplatin is a platinum-based chemotherapeutic that causes DNA crosslinking and apoptosis in rapidly dividing cells. - Fluorouracil (5-FU) is an antimetabolite that inhibits thymidylate synthase, disrupting DNA synthesis. - Folinic acid enhances the binding of 5-FU to thymidylate synthase, increasing its efficacy. - Bevacizumab is a monoclonal antibody targeting vascular endothelial growth factor A (VEGF-A), inhibiting angiogenesis required for tumor growth. This combination has been shown to improve progression-free survival and overall survival in patients with previously treated or untreated metastatic colorectal cancer[1][2][3][4]. It may also be used as first-line therapy in this setting.
