Fomepizole is a small molecule antidote primarily used to treat poisoning from methanol and ethylene glycol. It acts as a competitive inhibitor of the enzyme alcohol dehydrogenase, which catalyzes the initial steps in the metabolism of methanol and ethylene glycol into their toxic metabolites. By inhibiting this enzyme, fomepizole prevents the formation of harmful compounds such as formic acid (from methanol) and glycolate/oxalate (from ethylene glycol), thereby reducing metabolic acidosis, organ damage, and other severe complications associated with these poisonings. Fomepizole is administered intravenously and may be used alone or in combination with hemodialysis for severe cases[1][2][5][8].
