Fondaparinux is a synthetic anticoagulant classified as a selective factor Xa inhibitor. It is composed of five monomeric sugar units and structurally mimics the antithrombin III (ATIII) binding site found in heparin and heparan sulfate. Fondaparinux works by binding to ATIII, which enhances ATIII’s ability to inhibit activated Factor X (factor Xa), thereby interrupting the blood coagulation cascade and reducing thrombin generation. This results in decreased formation of blood clots. Fondaparinux is primarily indicated for the prevention and treatment of venous thromboembolism (VTE), including deep vein thrombosis (DVT) and pulmonary embolism (PE), especially after orthopedic or abdominal surgery, as well as for use in acute coronary syndromes such as unstable angina, non-ST segment elevation myocardial infarction (NSTEMI), and ST segment elevation myocardial infarction (STEMI)[1][2][3][8].
