Formoterol is a long-acting beta-2 adrenergic receptor agonist (LABA) used for the maintenance treatment of asthma and chronic obstructive pulmonary disease (COPD). Developed originally by Ciba-Geigy (now Novartis), it acts as a potent bronchodilator by selectively stimulating beta-2 receptors in the bronchial smooth muscle, which increases intracellular cyclic AMP levels and leads to muscle relaxation. Unlike other long-acting agents like salmeterol, formoterol has a rapid onset of action (1–3 minutes), making it unique as a LABA that can provide both quick and sustained relief (up to 12 hours). It is frequently utilized in fixed-dose combinations with inhaled corticosteroids (ICS) or long-acting muscarinic antagonists (LAMA). Beyond respiratory care, formoterol has been investigated for its metabolic effects, specifically its ability to increase resting energy expenditure and fat oxidation, suggesting potential therapeutic utility in treating obesity.
