This is a combination therapy consisting of three agents: - **Fosfomycin** is a broad-spectrum bactericidal antibiotic that inhibits bacterial cell wall synthesis by inactivating the enzyme UDP-N-acetylglucosamine enolpyruvyl transferase (MurA), thereby blocking peptidoglycan biosynthesis. - **Metronidazole** is an antibiotic and antiprotozoal agent that disrupts DNA synthesis in anaerobic bacteria and protozoa through reduction of its nitro group, leading to cell death. - **Recombinant human granulocyte-macrophage colony-stimulating factor (rhGM-CSF)** is a cytokine that stimulates the proliferation, differentiation, and activation of hematopoietic progenitor cells into granulocytes and macrophages. It enhances immune response by promoting growth and function of neutrophils and macrophages. The combination has been studied for intraperitoneal administration after abdominal surgery (e.g., appendectomy) to provide both direct antimicrobial action against aerobic/anaerobic pathogens (via fosfomycin/metronidazole) and immunomodulatory support to enhance host defense mechanisms (via rhGM-CSF). The mixture has demonstrated chemical stability for at least 24 hours with maintained antibacterial activity. Clinical studies indicate it can be safely administered intraperitoneally without serious adverse events[1][2][3][4].
