Fosinopril + hydrochlorothiazide is an oral fixed-dose antihypertensive combination containing fosinopril, an angiotensin-converting enzyme inhibitor prodrug, and hydrochlorothiazide, a thiazide diuretic. Fosinopril is converted to fosinoprilat, which inhibits angiotensin-converting enzyme and reduces formation of angiotensin II and aldosterone. Hydrochlorothiazide inhibits sodium and chloride reabsorption in the distal convoluted tubule, increasing natriuresis and reducing plasma volume. The combination is used for treatment of hypertension and is not intended as initial therapy.
