Fosinoprilat is the active metabolite of fosinopril, an angiotensin-converting enzyme (ACE) inhibitor used primarily for the treatment of hypertension and heart failure. Fosinopril itself is a prodrug that is rapidly hydrolyzed in the body to form fosinoprilat, which exerts its pharmacological effects. Fosinoprilat acts as a competitive inhibitor of ACE, blocking the conversion of angiotensin I to angiotensin II—a potent vasoconstrictor involved in blood pressure regulation and aldosterone secretion. By inhibiting this pathway, fosinoprilat reduces peripheral vascular resistance and lowers blood pressure, while also decreasing afterload on the heart in patients with heart failure[3][5][6]. Unlike some other ACE inhibitors that are eliminated primarily by renal excretion, both renal and hepatic pathways contribute to elimination of fosinopril/fosinoprilat[5].
