Foslevodopa is a prodrug of levodopa developed for the treatment of advanced Parkinson's disease. It is administered in combination with foscarbidopa as a continuous subcutaneous infusion (notably under the brand name Vyalev) to manage motor fluctuations in adults with advanced Parkinson’s disease. Foslevodopa is converted enzymatically into levodopa in the body, which then crosses the blood-brain barrier and is further converted to dopamine—a neurotransmitter deficient in Parkinson’s disease. The co-administered prodrug, foscarbidopa (a carbidopa prodrug), inhibits peripheral decarboxylation of levodopa, increasing its availability to the brain and reducing peripheral side effects such as nausea. This formulation allows for stable plasma levels and continuous dopaminergic stimulation[1][3][4][5][6].
