Overview

Fosmidomycin + clindamycin is an oral combination therapy investigated primarily for the treatment of uncomplicated *Plasmodium falciparum* malaria. Fosmidomycin is a phosphonic acid antibiotic that inhibits 1-deoxy-D-xylulose 5-phosphate reductoisomerase, an enzyme in the non-mevalonate pathway of isoprenoid biosynthesis, which is essential for malaria parasites but absent in humans. Clindamycin is a lincosamide antibiotic that inhibits bacterial protein synthesis by binding to the 23S rRNA of the 50S subunit of the ribosome; in malaria, it targets prokaryote-like ribosomes within the apicoplast organelle, disrupting parasite replication[4][5][6]. The combination has demonstrated high efficacy and good tolerability in clinical trials for pediatric and adult patients with *P. falciparum* malaria[4][6][8]. This regimen addresses late recrudescence seen with fosmidomycin monotherapy by leveraging synergistic action between both drugs[4].
