Four-factor prothrombin complex concentrate (4F-PCC) is a plasma-derived therapeutic containing concentrated levels of the Vitamin K-dependent coagulation factors II, VII, IX, and X, along with the anticoagulant proteins C and S. It is indicated for the urgent reversal of acquired coagulation factor deficiency induced by Vitamin K antagonist (VKA) therapy, such as warfarin, in adult patients with acute major bleeding or a need for urgent surgery. In the context of intracranial hemorrhage (ICH), 4F-PCC is used to rapidly normalize the International Normalized Ratio (INR) and achieve hemostasis more quickly and with a lower infusion volume than fresh frozen plasma (FFP), thereby potentially limiting hematoma expansion. The University of Utah has conducted clinical research (NCT02339129) to evaluate its efficacy and cost-effectiveness compared to FFP in both traumatic and spontaneous ICH.
