FOXO4-DRI (also known as Proxofim) is a synthetic, cell-permeable D-retro-inverso peptide designed as a selective senolytic agent to target and eliminate senescent cells. Developed by Cleara Biotech, a spinout from the University Medical Center Utrecht and Erasmus Medical Center, the peptide specifically disrupts the protein-protein interaction between the transcription factor FOXO4 and the tumor suppressor p53 (TP53). In senescent cells, FOXO4 sequesters p53 to prevent apoptosis; by competing for p53 binding, FOXO4-DRI releases p53, triggering selective apoptosis of these dysfunctional cells while sparing healthy tissue. Preclinical studies have shown that FOXO4-DRI can restore tissue homeostasis, improve renal function, and reverse chemotoxicity and age-related phenotypes in mice. It is also under investigation for its potential to target therapy-resistant, mutant p53-driven malignancies, such as triple-negative breast cancer and melanoma, as well as chronic conditions like pulmonary fibrosis.
