Overview

Fractional CO2 laser-assisted photodynamic therapy (PDT) is an enhanced dermatological treatment modality being investigated by Bispebjerg Hospital for the treatment of nodular basal cell carcinoma and moderate to severe actinic keratosis. The procedure involves pretreatment with a fractional CO2 laser to create microscopic treatment zones (MTZs) or channels in the skin, which facilitates deeper and more uniform penetration of the topical photosensitizing agent, methyl aminolaevulinate (MAL). Following an incubation period, the area is illuminated with red light, triggering a photochemical reaction that generates reactive oxygen species, leading to selective destruction of malignant or precancerous cells. This approach aims to improve the efficacy of conventional PDT, particularly for thicker or deeper lesions like nodular BCC.
