Overview

The fractional dose HPV vaccine refers to a reduced-dose regimen (one-fifth of the standard dose) of existing human papillomavirus (HPV) vaccines, specifically the bivalent Cervarix (GSK) and the nonavalent Gardasil 9 (Merck). This dosing strategy is being evaluated in a Phase IV clinical trial led by the University of Washington to assess its immunogenicity when administered via intramuscular or intradermal routes in adults aged 27-45. The vaccines are composed of virus-like particles (VLPs) derived from the L1 major capsid protein of various HPV types. Cervarix targets high-risk types 16 and 18 and utilizes the AS04 adjuvant system, while Gardasil 9 targets types 6, 11, 16, 18, 31, 33, 45, 52, and 58 with an aluminum-based adjuvant. The primary goal of investigating fractional doses is to potentially extend vaccine supply and reduce costs while maintaining protective antibody responses against HPV infection and related cancers.
