Frameshift peptide vaccines are a class of therapeutic and preventive cancer vaccines designed to elicit immune responses against tumor-specific neoantigens generated by frameshift mutations. These mutations commonly arise in cancers with DNA mismatch repair (MMR) deficiency, such as those seen in Lynch syndrome and microsatellite instability-high (MSI-H) tumors. Frameshift mutations in coding microsatellites lead to the production of novel peptides—frameshift peptides—that are not present in normal tissues and can be recognized as foreign by the immune system. The vaccines typically consist of synthetic peptides corresponding to these tumor-specific sequences, often formulated with adjuvants like Montanide ISA-51 VG or poly-ICLC to enhance immunogenicity. Clinical trials have demonstrated that these vaccines induce robust humoral and cellular immune responses without severe adverse events, making them promising candidates for both treatment and prevention of MMR-deficient cancers[1][3][4][5]. In veterinary medicine, similar approaches are being developed for cancer prevention in dogs[7].
