Framycetin + gramicidin + dexamethasone is a topical ophthalmic and otic combination product containing two antibiotics and a corticosteroid, used primarily to treat bacterial infections and associated inflammation of the external eye (e.g. conjunctivitis, blepharitis, keratitis, scleritis) and external ear (e.g. otitis externa). Gramicidin is a cyclic peptide antibiotic that inserts into bacterial cytoplasmic membranes to form ion channels, disrupting membrane integrity and leading to cell death, with predominant activity against Gram‑positive bacteria. Dexamethasone is a synthetic glucocorticoid that binds intracellular glucocorticoid receptors and modulates gene transcription to exert potent local anti‑inflammatory and immunosuppressive effects, reducing redness, swelling, and discomfort while the antibiotics eradicate susceptible bacteria. The combination is formulated as eye/ear drops for local action with minimal systemic absorption when used appropriately.
