Fu Zheng Hua Yu Jiang Zhu Tong Luo Fang (FZHY) is a Traditional Chinese Medicine (TCM) herbal prescription developed for the treatment of chronic kidney disease (CKD). It is a complex formulation consisting of nine traditional Chinese medicines: raw astragalus, *Rehmannia glutinosa*, Salvia, safflower, wine leeches, soil beetle, wine Scutellaria, wine rhubarb, and raw licorice. FZHY exerts its therapeutic effects by modulating the gut-kidney axis; it regulates the composition of intestinal microbiota (specifically reducing pathogenic genera such as *g_Monoglobus*, *g_Papillibacter*, and *g_Eubacterium_nodatum*) and decreases the levels of gut-derived harmful metabolites, including uremic toxins and secondary bile acids. This modulation leads to the restoration of the intestinal epithelial barrier (upregulating ZO-1, occludin, and claudin-1), suppression of systemic inflammation (reducing CRP, TNF-α, IL-6, and IL-1), and attenuation of renal tubulointerstitial fibrosis by inhibiting the deposition of extracellular matrix proteins like fibronectin, laminin, and collagens.
