Overview

Fucosylated cord blood cells are **umbilical cord blood-derived hematopoietic stem and progenitor cells that undergo ex vivo enzymatic fucosylation** (most commonly via fucosyltransferase-VI and GDP-fucose) prior to transplantation. This modification enhances surface fucosylation of cell adhesion molecules (such as the Sialyl Lewis X motif), thereby significantly improving their ability to bind P- and E-selectins on bone marrow endothelium. The resulting **increase in bone marrow homing efficiency leads to faster and more reliable neutrophil and platelet engraftment** after cord blood transplantation, especially in patients with high-risk hematologic malignancies. Clinical and preclinical studies show that these cells engraft more rapidly without compromising long-term multilineage reconstitution. The main indications under investigation are as an adjunct to cord blood transplantation for conditions like leukemia and lymphoma, aiming to overcome typical slow engraftment seen with unmanipulated cord blood[1][3][5][7][8].
