This treatment protocol refers to a specific myeloablative conditioning (MAC) regimen developed by New York Medical College for pediatric and young adult patients undergoing allogeneic hematopoietic stem cell transplantation (HSCT). The regimen is utilized for a broad range of malignant and non-malignant conditions, including leukemia, lymphoma, bone marrow failure syndromes, primary immunodeficiencies, histiocytosis, sickle cell disease, beta thalassemia, and inborn errors of metabolism. The protocol typically involves one of two intensive combinations: either fractionated total body irradiation (TBI) combined with thiotepa, cyclophosphamide, and rabbit antithymocyte globulin (ATG); or a non-radiation-based regimen consisting of busulfan, melphalan, and rabbit ATG. These regimens are designed to achieve complete marrow ablation and provide sufficient immunosuppression to facilitate the engraftment of CD34+ selected stem cells while minimizing the risk of graft-versus-host disease (GVHD).
