Fumaderm is an oral systemic therapy composed of a fixed-dose mixture of fumaric acid esters, specifically dimethyl fumarate and various salts of monoethyl fumarate (calcium, magnesium, and zinc). It is primarily indicated for the treatment of moderate to severe plaque psoriasis. The medication functions as an immunomodulator and anti-inflammatory agent; its active metabolite, monomethyl fumarate, activates the nuclear factor erythroid 2-related factor 2 (Nrf2) transcriptional pathway, which enhances the cellular antioxidant response and protects against oxidative stress. Additionally, it inhibits the translocation of nuclear factor kappa B (NF-κB) to the nucleus, leading to decreased expression of pro-inflammatory cytokines such as IL-12, IL-23, and TNF-α, and reduced expression of adhesion molecules. Developed by Fumapharm and later acquired by Biogen, Fumaderm has been a standard systemic treatment in German dermatological practice for several decades and served as the clinical precursor to the development of dimethyl fumarate for multiple sclerosis.
