Fumaderm initial is a branded oral formulation of fumaric acid esters (FAEs) developed by Almirall Hermal GmbH, primarily marketed in Germany. It serves as a lower-dose starter therapy (containing 30 mg of dimethyl fumarate plus monoethyl fumarate salts) designed to improve gastrointestinal tolerability during the initial weeks of treatment for moderate-to-severe plaque psoriasis, before patients transition to the full-strength Fumaderm formulation. The drug exerts its effects through immunomodulatory pathways, including the activation of the Nrf2 (nuclear factor erythroid 2-related factor 2) antioxidant response, induction of regulatory T cells, and shifting the T-helper cell profile from a pro-inflammatory Th1/Th17 response toward a Th2 response. Beyond its established use in psoriasis, Fumaderm initial is being investigated in Phase 2 clinical trials as an off-label treatment for therapy-resistant alopecia areata.
